19.12.23

A meeting of the Department of Ophthalmology was held, during which the report of a graduate student Andrii Mykolaiovych Zhuk was considered. The topic of his Ph.D. thesis is “Prediction of anatomical and functional results of combined surgical treatment of rhegmatogenous retinal detachment”. During the meeting, the details of the scientific work were discussed and possible options for improving the presentation of the results in the text of the dissertation were suggested.

The dissertation student along with his scientific supervisor Dmytro Gennadiiovych Zhaboiedov, M.D., Ph.D., D.Sc., head and professor of the Department of Ophthalmology made the following conclusions:

  1. Rhegmatogenous retinal detachment takes a share from 2% to 9% in the structure of primary disability due to diseases of the organ of vision. When conducting epidemiological observations, the prevalence of RSV is 6.9-18.2 cases per 100,000 population per year. The effectiveness of surgical treatment of retinal detachment is often evaluated only by the achievement of the anatomical effect, without taking into account the morphological and functional changes of the retina. RSV is often combined with age-related cataracts, which account for 90% of all cataracts. Its frequency in patients with retinal diseases is 15%. The combined operation of CSF and phacoemulsification for RBS is an effective and safe procedure.
  2. After SVR with tamponade of the vitreous cavity with C3F8 gas-air mixture, the average frequency of intraoperative complications was 25.0%, after 1 year, secondary cataracts were observed in 3.45% and IOL dislocation in 3.45%. After 1 year, uncorrected visual acuity increased statistically significantly by 2.3 times (t1 year= 4.9; p<0.01), maximum corrected visual acuity by 2.5 times (t1 year= 6.8; p<0.01), retinal light sensitivity in 10.2 times (t1 year= 9.2; p<0.01), local visual field defects decreased by 3.3 times (t1 year= 24.5; p<0.01) compared to the data before surgery.
  3. After SVR with tamponade of the vitreous cavity with silicone oil, the average frequency of intraoperative complications was 45.0%, after 1 year the following were observed: ophthalmic hypertension – 3.33%, keratopathy – 1.67%, IOL dislocation – 5.0%, secondary cataract – 13.33%, recurrence of retinal detachment – 1.67%, macular edema – 6.67%, epiretinal fibroplasia – 18.33%. After 1 year, uncorrected and maximally corrected visual acuity increased statistically significantly by 2 times (t1 year= 2.9, p<0.01; t1 year= 4.0, p<0.01), retinal photosensitivity by 2.1 times (t1 year= 6.6; p <0.01), local visual field defects decreased by 2 times (t1 year=15.3; p<0.01) compared to data before the operation.
  4. It was established that after 1 year of SVR with tamponade of the vitreous cavity with a C3F8 gas-air mixture, the thickness of the neurosensory retina in the foveolar zone increased by 22% (t1 year= 6.5; p<0.01), the thickness of the choroid decreased by 17%. (t1year= 4.9; p<0.01), and the number of eyes that did not have changes in the pigment epithelium and choriocapillary complex increased by 2.2 times (ϕ1year=2.4; p<0.01) and 3 times (ϕ1year= 7.5; p<0.01) with preserved or restored IS/OS layer compared to preoperative data. Risk factors after surgery that affected the thickness of the neurosensory retina in the foveolar zone were the age of detachment (H=45.57; p≤0.01) and the stage of PVR (H=46.25; p≤0.01).
  5. It was established that after 1 year of SVR with tamponade of the vitreous cavity with silicone oil, the thickness of the neurosensory retina in the foveolar zone increased by 26% (t1 year= 8.2; p<0.01), the thickness of the choroid decreased by 8% (t1 year= 2.1; p<0.01). p<0.05) and the RNFL index by 5% (t1 year=2.0; p<0.05), and the number of eyes with increased optical density of the pigment epithelium and choriocapillary complex increased by 2.7 times (ϕ1 year=2.2; p< 0.05) and with a preserved or restored IS/OS layer by 18% (ϕ1 year=1.8; p<0.05) compared to the data before surgery. Risk factors after surgery that affected the thickness of the neurosensory retina in the foveolar zone were the age of detachment (H=11.53; p=0.003) and the stage of PVR (H=10.007; p=0.006).
  6. Mathematical models have been developed that allow to determine with high accuracy the risk of non-improvement and/or worsening of the condition 1 year after surgical treatment of rhegmatogenous retinal detachment in combination with age-related cataract (area under the curve of operational characteristics AUC=0.82 (95% CI 0 .74-0.88) and AUC=0.91 (95% CI 0.84-0.95, respectively), which indicated a strong relationship with the established risk factors. The sensitivity of the models was 91.9% (95% CI 84.7% – 96.4%) and 90.9% (95% CI 81.3% – 96.6%), specificity – 57.9% (95% CI 33.5% – 79.7%) and 82.7% (95% CI 69.7% – 91.8%), respectively.

Good day, dear students!

This Friday, September 1, in the walls of his native NMU and the city of O.O. a large-scale event took place in Bogomolets – the MEDVOYAGE Freshman Festival .

The student scientific group of the Department of Ophthalmology represented the ZORKO station.

First-year students plunged into the world of ophthalmology and mastered basic practical skills in this field. Visual acuity was checked using a Rott apparatus, refractometry was performed – visual acuity was corrected using a set of lenses, ophthalmoscopy was performed – the presence of a “pink reflex” was checked, and the field of vision was examined using a perimeter.

Each of the students tried themselves in the role of both a patient and a doctor, conducted examinations and discussed interesting cases .

A lot of work has been done, for which we should thank our scientific director – Ryzhova Iryna Petrivna, as well as the student organizers of the ZORKO ophthalmology station: Alina Kovalova, Daria Kireeva and Stefiniv Oksana .

Thank you to every student who joined our station and shared your holiday with us! We look forward to seeing you at our group meetings!
