Abstract of the main results of the reporting stage (Year 2025)
During the reporting period, a comprehensive study of children with type 1 diabetes (T1D) was conducted under pre-war and wartime conditions. An electronic database was created, integrating clinical, biochemical, instrumental, and socio-psychological indicators, allowing assessment of individual risks for complications and monitoring disease progression.
Modern data analysis methods were applied, including data mining, machine learning algorithms, and the development of mathematical models for predicting complications, ensuring early detection of pathologies and personalized patient management.
The impact of war-related stress factors on the development of cardiomyopathy, sleep disorders, and psycho-emotional status in children was investigated. Specific biochemical markers of oxidative stress and early indicators of tubular kidney damage (KIM-1) were identified, enabling detection of subclinical stages of diabetic nephropathy. A correlation between KIM-1 levels, oxidative stress markers, HbA1c, and disease duration was established, forming the basis for predictive modeling of complications.
A prototype of an automated information system for monitoring the health of children with T1D was developed, integrated into the electronic health system, allowing timely treatment adjustments and reducing the risk of disability.
New diagnostic, therapeutic, and rehabilitation schemes for children with stress-related disorders were implemented, contributing to reduced symptom duration, improved psycho-emotional well-being, and enhanced quality of life.
The results have high scientific and practical value for medicine, the economy, and Ukrainian society, supporting national security and providing a foundation for further research and the integration of personalized approaches into pediatric practice.
The scientific work of the department is aimed at solving the most urgent problems of clinical pediatrics and is carried out in close cooperation with research institutions of the National Academy of Sciences, laboratories that allowes to make a significant contribution to the development of pediatric science in our country.
Therefore, in collaboration with Institute of Physiology the role of genetic polymorphisms of different gene ensembles in the genesis of type 1 diabetes, as well as the interrelation of sleep-wake cycle and obesity were studied. Together with the Institute of Hydromechanics of the National Academy of Science of Ukraine a computer complex for phonospirographic diagnostics of respiratory diseases in children was developed, the diagnostic efficiency of the method in the diagnosis of pneumonia, monitoring of the treatment efficiency conducted. In collaboration with the National University of Life and Environmental of the National Academy of Science of Ukraine, State Institution «Institute of Urology of the National Academy of Medical Science of Ukraine», R.E. Kavetskiy Institute of Experimental Pathology, Oncology and Radiobiology the ability of biofilm, different bacteria and their role in the emergence and course of urinary tract infections were studied. In recent years, the department has widely implemented modern methods of instrumental diagnostics, some of which are simply unique. First of all, the method of estimation of the state of the autonomic nervous system with the help of cutaneous sympathetic potentials and the method of electroneuromyography, computer spirography for assessment of the function of external respiration and respirosonography in children, as well as remote computer thermography for the diagnosis of various diseases were introduced. Daily monitoring of blood pressure, ECG and assessment of heart rate variability in children, assessment of urodynamics of the lower urinary tract with the help of computer neuroelectrophysic complex were established. Currently, the department is carrying out an initiative research work “Development of criteria for diagnosis and substantiation of therapeutic tactics of metabolic disorders in pathology of the cardiovascular, endocrine and urinary system in children” (2018-2020). New methods of diagnostics, treatment and prevention of childhood diseases, developed by the staff of the department, are protected by copyright certificates on inventions and patents, as well as innovative proposals. Materials of scientific developments are the basis of methodological recommendations and information sheets, issued for implementation in children’s health care and preventive health care institutions.
