3. Diploma of Higher Medical Education (original and copy)
4. Certificate of Medical Specialist (original and copy)
5. Certificates of completed specialization or advanced training cycles (copies)
6. Qualification category certificate (copy)
7. Marriage/divorce certificate if documents were issued under a different surname (copy)
8. Copy of employment record book certified by the HR department of the employing medical institution
9. Document folder
10. Two 3×4 cm photographs (for specialization trainees)
11. Personal trainee record card
12. Educational services contract for enrollment on a fee-paying basis (2 copies)

Additionally, for enrollment on a state-funded basis:

14. Referral from the Department of Health indicating the duration of specialization training
