History of department

The course on anesthesiology and emergency medicine before establishment of department of anesthesiology and emergency medicine was taught in department of faculty surgery named after academician Krymov O.P.; department of hospital surgery; department of surgery in dental faculty. The first teachers, who were doctors-anesthesiologists due to occupation and who started teaching anesthesiology and emergency medicine at surgical department of KMI until establishment of department of anesthesiology and emergency medicine in KMI, were: Zhalko-Tytarenko V.P. – department of hospital surgery; Antonova G.S. – department of faculty surgery named after academician Krymov; Deputat A.Yu. – department of surgery in dental faculty. The official separation of independent department of anesthesiology and emergency medicine was made on 16.07.1974. The first head was professor Leonard Petrovych Chepkyy – one of founders for Ukrainian school of anesthesiology, emergency medicine and intensive therapy, Laureate of State Prize of Ukraine, honorary scientist and technician of Ukraine, and the first employees at department were assistant professor V.F. Zhalko-Tytarenko and assistants Koval D.A., Deputat A.Ye., Antonova G.S., etc. Since organization of department the main direction in its scientific activity was the study on problems of anesthesia and postoperative intensive therapy at patients with peritonitis and pancreatitis. The candidate’s dissertation, defended in 1980 by Tsertiy V.P., was devoted to that topic. The result from performed researches was the implementation of such new methodologies as hemodilution and artificial diuresis, artificial localized hypothermia, methods on balanced correction for disorders in water-electrolytic balance and acid-base balance, protein nitrogen metabolism, rheological properties of blood that allowed decreasing the lethality at patients with peritonitis by 12% into practical health care. At the same time with the problem on peritonitis they studied the methods on prevention and treatment of acute respiratory insufficiency, developed and implemented the methods on spontaneous breathing with positive support on exhalation, which allowed reducing the frequency in postoperative pulmonary complications almost by two times, into clinical practice. Further they started deeply studying the peculiarities for course and intensive therapy of peritonitis at patients of old and very old age. Based on materials from those works in 1987 they published monograph “Intensive therapy for surgical patients of old and very old age”. The issues on anesthetic provision of surgical interferences in abdominal surgery were actively studied by department, in 1990 the results were generalized in the form of monograph “Anesthesia and postoperative intensive therapy of patients with adiposity”. While developing the methodologies on intensive therapy, the great attention was also paid to the choice of methods on postoperative anesthesia. Taking into consideration the great frequency in development of stress postoperative ulcers and gastric hemorrhage, the collective of department was engaged into study of reasons and pathogenesis of their development, as well as methods on prevention and treatment of those complications. The individual problem, into which the collective of department was engaged, was the work, devoted to methods on intensive therapy of patients with leptospirosis. It was of special importance, taking into consideration the significant increase in frequency of chases for leptospirosis and high lethality at it at that time. As a result from application of methodologies on extracorporeal detoxification they managed to reduce the lethality in that group of patients by 47%. At the same time the employees at department are engaged in issues on intensive therapy of critical states. The collective of department was engaged not only into issues on anesthesiology and intensive therapy but problems on emergency and immediate medical aid. In 1987 due to the task from MHC of USSR together with departments of anesthesiology and emergency medicine in the Ist Moscow medical institute and Dnipropetrovsk medical institute they established all-Union program and methodic developments for training of doctors-interns with specialty “anesthesiology and emergency medicine”, and since 1988 the collective of department starts training doctors-interns. Since 1990 until 2000 the department is managed by Doctor of Medicine, professor Dubytskyy Anatoliy Yefymovych. Since 1991 the department was reorganized into department of anesthesiology, emergency medicine and disaster medicine. The scientific activity of department at that period is characterized by its clinical physiological direction, advanced research on components of pathophysiological disorders, which accompany the critical states, orientation to implementation of the newest achievements of science into practice, close relation with practical medicine. Since 2002 the department was managed by Doctor of Medicine, professor Felix Semenovych Glumcher. Since 2004 the department has the name “Department of anesthesiology and intensive therapy”. The new stage in development of department is characterized by active work of the head and all employees within a wide range of scientific interests, practical medicine, and academic methodological direction. The department takes an active part in the activity of Association of anesthesiologists of Ukraine, which President is Felix Semenovych, publications of periodical scientific editions: journals “Pain, anesthetization and intensive therapy” and “Medical abstractive review journal”, guidelines, textbooks and handbooks. The corresponding sections describe the activity of department at present stage in details.
