Department of otorhinolaryngology

Scientific work

Responsible for scientific work: Associate Professor Shevchuk Yurii Viktorovych phone 0506018944, email yuralor@ukr.net

Main areas of work: acute and chronic pathology of the ear, acute pathology of the nose and paranasal sinuses, acute and chronic pathology of the pharynx and larynx, injuries of the ENT organs and adjacent areas.

During 2020 – 2022, the department carried out the initiative and search topic “Development and improvement of new technologies in the diagnosis and treatment of acute and chronic diseases of the head and neck”

The most important specific scientific results were obtained during the implementation of the completed planned research work:

  1. The use of a modern regimen of antineuritic therapy in patients with sensorineural hearing loss, depending on the cause, timing and extent of damage to the hearing organ, is effective in most patients and, as a rule, leads to the restoration of hearing function with early initiation of therapy (up to 14 days).
  2. Taking into account the examination data in the preoperative and postoperative periods, laryngeal reinnervation should be considered as an effective method of surgical correction of dysphonia of unilateral obduction paralysis of the larynx.
  3. The use of the tissue coblation method for surgical treatment of patients with decompensated form of chronic tonsillitis, hypertrophy of adenoids, palatine tonsils, vasomotor and hypertrophic rhinitis is effective, safe and cost-effective. There is a rapid healing of the postoperative wound against the background of a reduction in the duration of surgery and anesthesia, a low percentage of complications, a decrease in the length of hospital stay to 1 bed-day and drug therapy.
  4. Surgical treatment of obstructive sleep apnea syndrome by surgical intervention according to a proprietary technique is effective and safe, improves the quality of life of patients and avoids typical complications, provided that there is an adequate preoperative examination.
  5. In a clinical and experimental study, the presence of immunological changes and changes in auditory function (sensorineural hearing loss) using UAE and SWR was proved both in animals with simulated autoimmune thyroiditis and in patients with this pathology, which makes it possible to identify early manifestations of SNP and predict their appearance in the future at the preclinical stage.
  6. In the statistically significant (the majority of patients who developed pathology of the maxillary sinus in the remote postoperative period, in the preoperative period, there was an increase in the level of MMP-9 in saliva (19 patients, 90.5%) and in the removed material – the membrane of the maxillary sinus cyst (17 patients, 81.0%). An increase in the activity of MMP-9 in the membrane of the maxillary sinus cyst is statistically significantly correlated with an active inflammatory process in the bone tissue of the upper jaw around root of the tooth and amounted to 83% (p <0.05).
  7. It has been established that the optimal method of treatment of children with neuro-vegetative form of vasomotor rhinitis is the performance of submucosal vasotomy of the lower turbinates by radio wave method. This technique allows you to perform the operation almost bloodlessly, on an outpatient basis, without the use of tampons. The use of this technique made it possible to restore nasal breathing in 98% of patients, while reducing the patient’s stay in the hospital and the absence of the need for further rehabilitation.
  8. Improved tactics of providing care to patients with injuries of the ENT organs, in particular gunshot, military combined, acubarotraumas, which allows us to provide assistance to these patients as quickly and efficiently as possible, accelerates treatment and recovery, reduces the number of complications, and contributes to the fastest rehabilitation.
  9. New methods of closing nasal septal perforations using a platelet clot and a combined fascial-fat flap have been developed to close defects in 83% of patients and restore the normal functioning of the nasal cavity and the quality of life of patients.
  10. The scheme of examination and treatment of patients with allergic rhinitis was improved, which made it possible to establish a diagnosis in 98% of patients, reduce the symptoms of the disease, improve the quality of life of patients, both by reducing symptoms and improving compliance with therapy.
  11. Treatment regimens for ENT pathology in patients with COVID-19 have been developed, which has improved the results of comprehensive treatment of these patients, accelerated recovery and reduced the number of complications.
  12. The effectiveness of conservative treatment of inflammatory diseases of the ENT organs in adults and children with various pathologies, including acute and chronic rhinosinusitis, adenoiditis, acute and chronic tonsillitis, sensorineural hearing loss has been established, which has improved the results of treatment of these patients, accelerated recovery and reduced the number of complications.

Currently, the applied research topic “The role of gene polymorphism in the development and progression of sensorineural hearing loss”, state registration number 0124U000820, scheduled for 2023-2025, is being carried out.
