TRAINING AND METHODOLOGICAL WORK
Requirements for the Level of Education for Individuals Eligible to Begin Training in Relevant Specialty Programs and Their Learning Outcomes
Applicants must hold a Master’s degree (or a Specialist degree for graduates prior to 2021) in the field of knowledge “22 Health Care,” specializing in “Medicine” or “General Medicine” (in the field of “Medicine”).
Format of Internship
The internship must be full-time.
Monitoring and Evaluation
As per the order of the Ministry of Health of Ukraine dated June 22, 2021, No. 1254 “On Approval of the Regulations on Internship and Secondary Medical (Pharmaceutical) Specialization,” registered with the Ministry of Justice of Ukraine on August 17, 2021, under No. 1081/36703. The educational part of the internship includes an annual and final differentiated assessment
Level of Mastery of Practical Skills
The level of mastery for each competency or practical skill is assessed on a scale from “A” to “D”:
- A: Knows, describes.
- B: Performs, demonstrates under supervision.
- C: Performs, demonstrates independently.
- D: Performs, teaches, or supervises younger interns in demonstration and performance.
Schedule of the Educational and Practical Parts of the Obstetrics and Gynecology Internship
- Duration of Internship Training: 33 months (5148 hours), which equals 171.6 ECTS credits.
- Educational Component: (Higher or Postgraduate Education Institutions) 9 months (1404 hours, 46.8 ECTS credits), 3 months annually (15.6 ECTS credits per year).
- Practical Component: (Training Bases) 24 months (3744 hours).

- Ability to solve specialized tasks and problems in professional activities in the field of health care, specializing in “Obstetrics and Gynecology.”

- Knowledge of Ukrainian health legislation and its application in daily practice.
- Ability to engage in abstract thinking, search, analyze, synthesize, and process information, including from various sources related to their professional field.
- Ability to practice professionally, adhering to academic integrity, ethical norms, and respect for cultural, ethnic, religious, gender, and social diversity, while maintaining patient confidentiality and dignity.
- Effective interpersonal communication, both orally and in writing, with patients, their families or representatives, colleagues, and interdisciplinary teams.

- Ability to collect medical information about a patient and analyze clinical data.
- Capability to interpret laboratory and instrumental test results.
- Ability to diagnose conditions, including preliminary, clinical, final, and associated diagnoses, as well as emergency conditions.
- Ability to determine tactics, methods, and provide emergency medical and pre-medical care following international protocols.
- Knowledge of the indications, contraindications, and side effects of medications used in medical practice, including adverse reactions to systemically administered drugs by patients.
Professional Competencies
- Skills in gathering patient and family history, including social aspects, conducting accurate clinical examinations of vital signs, internal and external genital organs, and abdomen, and adequately interpreting results.
- Ability to diagnose, assess, investigate, monitor, and interpret data on common obstetric and gynecological conditions.
- Ability to conduct timely and appropriate tests, such as microbiological samples, laboratory tests, and radiological examinations, and interpret results in collaboration with colleagues (e.g., radiologists) regarding clinical data to form a differential diagnosis.
- Proficiency in providing basic therapeutic interventions, including the safe and appropriate administration of oxygen, medications, therapy, blood products, circulatory support, and urinary catheterization.
- Ability to assess, prevent, and treat pain.
- Ability to recognize and triage acutely ill patients and initiate appropriate treatment, including for septic patients, postpartum complications, and resuscitation cases.
- Ability to maintain effective communication with patients and families based on principles of shared decision-making and informed consent, accurately document this communication, and engage in teamwork with effective communication within the medical team.
- Skills in perioperative care, including determining indications and contraindications for surgeries, surgical risks, indications for blood transfusion, recognition of postoperative complications, and indications for resuscitation.
- Ability to assess the impact of gynecological diseases on sexual function, evaluate sexual function, address possible negative sexual experiences, and identify the effects of sexual violence on gynecological pathology and behavior.
- Skills in assessing the biopsychosocial aspects of obstetric and gynecological conditions.
- Ability to diagnose early pregnancy, its location, embryo viability, complications, and provide basic therapeutic interventions, as well as indications for further examinations.
- Ability to develop a plan and management tactics for normal pregnancy, labor, and postpartum period by making informed decisions based on current clinical protocols and guidelines.
- Competence in assessing the general condition of newborns by making informed decisions based on clinical protocols and guidelines, while adhering to ethical and legal standards.
- Ability to establish a final clinical diagnosis through informed decision-making and logical analysis of subjective and objective data, including clinical and additional investigations, and performing differential diagnosis based on clinical protocols and guidelines.
- Ability to diagnose pregnancy complications, develop management plans and tactics, and determine indications for delivery based on medical indications by making informed decisions according to clinical protocols and guidelines.
- Skills in diagnosing labor, developing a plan and management tactics for uncomplicated labor following current clinical protocols and guidelines.
- Ability to provide assistance in complicated labor (preterm birth, operative delivery, multiple pregnancy, and breech presentation) following clinical protocols and guidelines.
- Ability to diagnose fetal distress during labor and determine labor management tactics according to clinical protocols and guidelines.
- Ability to determine indications and perform planned, repeat, and emergency cesarean sections following clinical protocols and guidelines.
- Ability to diagnose and provide conservative and surgical care for third-stage labor bleeding, early and late postpartum hemorrhage according to clinical protocols and guidelines.
- Ability to diagnose and provide surgical care for birth canal injuries following clinical protocols and guidelines.
- Ability to diagnose, determine treatment indications, and ensure treatment for pathology of the external genital organs, intrauterine pathology, and uterine and adnexal diseases.
- Competence in determining indications for treatment and consulting on treatment options, as well as providing conservative treatment for common gynecological pathologies.
- Ability to determine treatment indications, consult on treatment options, and provide surgical treatment for common gynecological pathologies.
- Skills in advising on contraception for healthy adults and patients with health problems or coexisting diseases.
- Ability to insert a diaphragm/cervical cap, intrauterine contraceptive, or subdermal implants.
- Skills in assessing subfertility and fertility in men and women.
- Ability to diagnose menstrual cycle disorders (primary amenorrhea, secondary amenorrhea, oligomenorrhea), identify their causes, and determine treatment options.
- Ability to identify urogynecological issues and pelvic floor dysfunction, determine treatment indications, and provide treatment, including surgical intervention when necessary.
- Ability to diagnose precancerous gynecological conditions through cervical screening, colposcopy with biopsy, discuss treatment options, and determine specific treatment indications.
- Competence in identifying and diagnosing gestational trophoblastic disease, ovarian carcinoma, cervical carcinoma, endometrial precancerous or malignant conditions, recurrence or progression of gynecological oncological conditions.
- Skills in diagnosing and treating sexually transmitted infections (STIs) in adults, adolescents, and children in the prepubertal and pubertal periods.
- Ability to provide emergency care for trauma to the vulva, vagina, perineum, and rectum in adolescents and children during the prepubertal and pubertal periods.
- Skills in conducting clinical examinations and screening for breast pathology.
Professional Activity in Obstetrics and Gynecology
Professional activity encompasses all tasks and aspects necessary for effective practice in providing patient care. For instance, the professional competence of performing a cesarean section includes not only the technical skills for the surgery itself but also the decision-making process leading up to the surgery, communication with the patient, teamwork in the operating room, and more. Entrusting an intern with professional tasks signifies that the intern is deemed competent in all facets of the activity, enabling them to perform the task independently.
The intern’s training process, aimed at achieving independent practice, relies on active involvement in patient care and providing formative feedback between clinical supervisors and the intern to assess progress in gaining competency. Over time, these clinical activities will lead to increased competence, and supervision will be adjusted accordingly. During the initial stages of training, the intern’s activities should be closely monitored, with clinical supervisors present during specific tasks to discuss the procedure or intervene when necessary.
Competency development progresses through four levels, from observing the clinical supervisor performing a task (Level A) to the intern performing the task independently (Level D).
Stages of Competency Development
- Level A: Intern observes the supervisor performing a specific task.
- Level B: Intern performs the task under direct supervision.
- Level C: Intern performs the task with indirect supervision or occasional guidance.
- Level D: Intern performs the task independently.
Simulation Training
Simulation-based training is an integral part of the obstetrics and gynecology curriculum to help interns acquire essential skills before applying them in clinical practice. Emergency obstetric situations are rare, making simulated training crucial for preparing interns to handle such emergencies. Simulation training also fosters the development of both individual skills and teamwork.
Simulation training involves the use of “mannequins, simulators, trained actors, real and virtual environments, and social situations that simulate problems, events, or conditions that occur during professional activities.” This training can take place in any setting.
Independent Practice
Achieving competency at the level of independent practice means that the intern should be able to provide care or perform procedures without the need for supervisory intervention.
List of Practical Obstetric Stations for Certification
As part of the final certification process for assigning (or confirming) the title of “Specialist Physician” (Obstetrician-Gynecologist), practical skills are evaluated in a range of obstetric stations. This certification ensures that interns have acquired the necessary hands-on experience and are ready for independent practice.
