LEGALIZATION OF EDUCATIONAL DOCUMENTS

The International Relations Department provides advisory and organizational services on legalization of documents on education abroad to graduates of the Bogomolets National Medical University.

 

The International Relations Department prepares the following documents:

• certificate confirming the training and issuance of a specialist diploma (Ukrainian language);
• certificate confirming the training and issuance of a master’s degree (Ukrainian language);
• fill in the verification form (ECFMG, other) (in two languages: Ukrainian and English);
• certificate confirming training in internship (Ukrainian language);
• extract from the thematic plan and internship program (Ukrainian language);
• certificate confirming training in clinical residency (Ukrainian language).

 

The International Relations Department also provides additional services:

• approve copies of documents on education.

 

RULES FOR ORDERING AND RECEIVING DOCUMENTS

 

If the Graduate personally submits a package of documents to the International Relations Department:

• Application (example at the International Relations Department);
• Graduate’s passport (1 page and residence permit);
• Copies of educational documents (diploma, diploma supplement, certificate of completion of internship, clinical residency – depending on the type of certificate to be ordered).

 

If the Authorized Representative submits a package of documents to the International Relations Department:

• Application (Annex 1);
• copy of the passport of the Authorized Representative and Graduate (1 page and residence permit);
• notarized power of attorney (with translation into Ukrainian language);
• copies of educational documents (diploma, diploma supplement, certificate of completion of internship / clinical residency – depending on the type of certificate to be ordered).

 

Duration of documents reviewing – one calendar month from the date of writing the application

 

SERVICES ARE TO BE PAID.

 

Documents on education are issued personally to the Graduate or their Authorized Representative (in the presence of a notarized power of attorney).
