PROCEDURE TO EXECUTE MEDICAL DOCUMENTATION IF A STUDENT IS ILL

According to the Order by MHC of Ukraine, dd. 14, February, 2012 under No 110, registered in Ministry of Justice of Ukraine, dd. 28, April, 2012 under No 661/20974 “About approval of forms for primary accounting records and Instructions to fill them in, which are used at health care establishments regardless of a pattern of ownership and subordination”. In case of temporary disability of a student they execute “Statement about temporary disability of student at educational establishment of I-IV accreditation levels, about disease, quarantine and other reasons for absence of child, who attends the general education establishment, pre-school educational establishment (hereinafter referred to as Form No 095/o).
Form No 095/o is filled in by doctor of health care establishment in case of temporary invalidity of students at educational establishments of I-IV accreditation levels, about disease, quarantine of child, who attends the general education establishment, pre-school educational establishment.
According to section 16 of mutual order by Ministry of education of Ukraine and Ministry of health care of Ukraine, dd. 06, June, 1996 No 191/153, registered in Ministry of justice of Ukraine on 24, June, 1996 No 325/1350 “About approval of Provisions about academic leaves and repeated study at higher schools” – sick-vouchers (form No 095/o) are certified at medical preventive establishment, which serves a higher school (Kyiv city students’ polyclinic), and are submitted to a higher school (dean’s office of the corresponding faculty) during a week after the treatment is finished.
A student of Bogomolets NMU may receive a sick voucher due to the place of residence or at students’ polyclinic in c. Kyiv (Politekhnichna str., 22, office 310).
A VALID REASON FOR ABSENCE AT CLASSES DUE TO UNEXPECTED FAMILY CIRCUMSTANCES IS CONSIDERED BY THE CORRESPONDING DEAN RECTOR OR HIS DEPUTY ON INDIVIDUAL BASIS.
